Description:
1. The subject of the order is the purchase and delivery of medical equipment - an ultrasound device and an EEG device for the Neurology Department with the Stroke Subunit of the Multi-Specialty District Hospital in Tarnowskie Gry. The order was divided into 2 parts: Part A Apparatus Ultrasound2. The detailed scope of the subject of the order, the description required by the Ordering Party in terms of implementation and acceptance are specified in: 1) the offer form with a description of the subject of the order, Appendix No. 1 to the SWZ2) draft provisions of the contract, Appendix No. 3 to the SWZ.3. Required period guarantee and warranty for the completed subject of the contract is a minimum of 24 months from the date of signing the fault-free delivery and acceptance protocol by the Ordering Party, the User (i.e. WSP SA) and the Contractor.4. The ordering party demands from the contractor specified in the description of the item the order constituting Appendix No. 1 to the SWZ of the said evidence confirming that the offered deliveries meet the requirements specified by the Ordering Party.5. The contractor submits the evidence together with the offer.6. If the contractor fails to submit the said evidence or the complex evidence in question is incomplete, the contracting authority will request its submission...
