Description:
1. The subject of the order is the purchase of medical equipment (ultrasound, mammography, system for intraoperative tissue radiography, laboratory incubator) for Copernicus PL Sp. z o. o. The subject of the order consists of 5 parts:
1)Part No. 1 ultrasound,
2)Part No. 2 ultrasound,
3)Part No. 3 Mammography,
4)Part No. 4 System for intraoperative tissue radiography,
5)Part No. 5 Laboratory incubator.
2. Detailed scope of the subject of the order, list of devices, quantities and the method of order execution are included in the annexes to the SWZ, namely:
No. 1A to SWZ Price form,
No. 1B to SWZ Description Subject orders,
No. 3A to SWZ - Draft contract for a one-off delivery (applies to parts 1, 2, 4, 5),
No. 3B to SWZ Draft contract for the supply of a mammogram (concerns part 3).
The contractor by completing the indicated items in Annex No. 1B to the SWZ- OPZ confirms compliance with the requirements specified by the Ordering Party.
3. The offered items must be complete, of a high standard both in terms of quality and functionality, and free from material, construction and legal defects.
4.The offered subject of the order must be approved for trading and use in accordance with applicable law.
5. The subject of the order must be in accordance with the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws 2022.974 of 2022/05/09).
