- Olaparib 1. The subject of the order is the successive deliveries of Olaparib to the Chemotherapy Department. 2. The description of the subject of the order is included in Annex No. 2 to the SWZ (product and price form). 3. Code defining the order according to the Common Procurement Vocabulary CPV: 33600000-6 Pharmaceutical products, 33652100-6 Anticancer agents. 4. Place of order fulfillment: ul. Grenadierów 51/59, 04-073 Warszawa. 5. Successive deliveries of the subject of the order are to be made to the Hospital Pharmacy located in Pavilion III of the Hospital on the ground floor and to the warehouse located there in the basement of the Hospital, and each time the Ordering Party will indicate if the place of delivery is a warehouse. 6. The subject of the order (range, pharmaceutical product) will be collected located in rooms accessible to the Contractor's employees (suppliers, couriers). 7. Deliveries can be made between 8:00 a.m. and 3:00 p.m. on business days, i.e. from Monday to Friday, excluding public holidays. 8. Detailed conditions execution of deliveries, including access to the Ordering Party's buildings, is specified in the draft contract constituting Annex No. 4 to the SWZ. 9. Required deadline for partial deliveries: 3 business days from placing the order. 10. Minimum shelf life of the assortment: 12 months from delivery date. 11. The ordering party will be obliged to use at least 75% of the gross value of the awarded order. The use will depend on the number of patients reporting to the Ordering Party...
