Description:
1. The subject of the order is the purchase of PET/CT and SPECT devices for Copernicus PL Sp. z o. o. The subject of the order consists of 1 part, which is divided into 2 tasks:
1) Task no. 1 PET/CT device (1 pc),
2) Task No. 2 SPECT device (1 pc),
2. Detailed scope of the subject of the order, the list of devices, quantities and the method of order execution are included in the annexes to the SWZ, namely:
No. 1A to SWZ Price form,
No. 1B to SWZ - Description of the Subject of the Order,
No. 3 to SWZ Draft contract.
Contractor filling out the indicated items in Annex 1B to the SWZ - OPZ confirms the fulfillment of the requirements specified by the Ordering Party in tasks No. 1 and 2.
3. The offered items must be complete, of a high standard both in terms of quality and functionality, and free from material defects, construction and legal.
4. The offered subject of the order must be admitted for trading and use in accordance with applicable law.
5. The subject of the order must be in accordance with the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws 2022.974 of 2022/05/09).
6.In all the provisions of the SWZ and its annexes, which indicate trademarks, parameters, sources or a specific process that characterizes the products supplied by a specific contractor of origin (names of producers or devices), in accordance with Art. 99 section 5 and when the subject of the order is described by...
