Description:
1. The subject of the order is: purchase of Nivolumab.
2. The description of the subject of the order is included in Annex No. 2 to the SWZ (product and price form) - Nivolumabum concentrate for preparing a solution for infusion.
3. Successive deliveries of the subject of the order are to be made to the Hospital Pharmacy located in Pavilion III of the Hospital on the ground floor and to the warehouse located in the basement of the Hospital, and each time the Ordering Party will indicate if the place of delivery is a warehouse.
4. The subject of the order will be collected in rooms available to the Contractor's employees (suppliers, couriers).
5. Deliveries can be made between 8:00 a.m. and 3:00 p.m. on business days, i.e. from Monday to Friday, excluding public holidays.
6. Detailed conditions for the implementation of deliveries, including access to the Ordering Party's buildings, are specified in the draft contract constituting Annex No. 4 to SWZ.
