Description:
1. The subject of the order is: purchase of pharmaceutical products - Abirateroni acetas and Bleomycinum. 2. The description of the subject of the order is included in Annex No. 2 to the SWZ (assortment and price form). 3.Code specifying the order according to the Common Dictionary Orders CPV: 33600000-6 Pharmaceutical products, 33652100-6 Anticancer agents. 4. Place of order fulfillment: ul. Grenadierów 51/59, 04-073 Warszawa. 5. Successive deliveries of the subject of the order are to be made to the Hospital Pharmacy located located in Pavilion III of the Hospital on the ground floor and to the warehouse located there in the basement of the Hospital, and each time the Ordering Party will indicate if the place of delivery is a warehouse. 6. The subject of the order will be collected in rooms accessible to the public the Contractor's employees (suppliers, couriers). 7. Deliveries can be made between 8:00 a.m. and 3:00 p.m. on business days, i.e. from Monday to Friday, excluding public holidays. 8. Detailed conditions for the implementation of deliveries, including access to the Employer's buildings, is specified in the draft contract constituting Annex No. 4 to the SWZ.
