The order was financed as part of the implementation of the task being the subject of the recruitment: Competition No. FM-STI.01.REHABILITACJA.ROBOTY.2023 for the selection of applications for co-financing regarding the implementation of the task consisting in supporting the development of innovative solutions for providing health care servicesin the field of equipping medical entities with robotic devices for rehabilitation from the resources of the therapeutic and innovative subfund of the Medical Fund. 1. The subject of the order is the purchase of equipment for the needs of SP ZOZ WSS No. 3 in Rybnik
2. A detailed description of the subject of the order is included in the attachmentattachments:
1) Offer form (package 1-4) - Annex 1 to the SWZ, 2) Price form (package 1-4) - Annex No. 1.1 to the SWZ, 3) Technical specification (package 1-4) - Annex No. 1.2 to SWZ, 4) Terms of quality and service guarantee (package 1-4) - Annex No. 1.3 to SWZ, 5) Draft provisions of the contractowe, which will be introduced into the content of the contract/contract provisions (package 1 to 4) - Annex No. 2 to the SWZ.
3.The Ordering Party allows the possibility of submitting partial offers, therefore the subject of the order is divided into 4 separate parts (packages), including: Diagnostic and rehabilitation treadmill - 1set, Automated system for functional training - 2 sets, Robotic mobile splint for hip and knee joint exercises - 2 sets, Robot for hand and finger therapy - 1 set.
