Description:
1. The subject of this order is the purchase of IR-192 sources for three Flexitron HDR cameras with service for the National Oncology Institute of Maria Skłodowska-Curie-National Research Institute of the Branch in Gliwice.
2. Description of the subject of the contract and the conditions of implementationOrders are included in the:
- assortment-price specification- Annex 2 to SWZ,
- Description of the subject of the contract- Annex 3 to SWZ,
- Described provisions of the contract, which will be introduced into the content of the contract- Annex 5 to SWZ.
3. Offers not containing a full rangethe subject of the order will be rejected.
4. The Employer allows the possibility of submitting equivalent offers - the equivalence description was included in Chapter IV SWZ.
5. The contracting authority does not provide for an electronic auction and supplementary orders.
6. Information on the joint application of contractorsfor the awarding of the contract were included in Chapter IX SWZ.
7. The contracting authority provides for the possibility of amending the provisions of the concluded contract (so -called contract changes based on Article 455 (1) (1) of the Act) in relation to the content of the offer, on the basis of which the contractor was selected, in accordance with the terms concludedIn Annex 5 to SWZ.
