- The subject of the order is: Purchase of support for NCBR Group employees consisting in providing group insurance to employees, associates of NCBR, NCBR+, NCBR ASI, IDEAS NCBR and their family members:
1. in the basic scope for all employees (contribution financed by the Ordering Party) V0 package:
a) Death of the insured person;
b) Death of the Insured as a result of accident;
c) Death of the Insured as a result of an accident at work;
d) Death of the Insured as a result of a traffic accident;
e) Death of the Insured as a result of a traffic accident at work;
f) Death of the Insured as a result of a heart attack/stroke;
g)Total incapacity work;
h) Permanent damage to health due to accidents;
i)Permanent damage to health resulting from a heart attack/stroke;
j)Serious illness;
k) Hospital treatment of the Insured, a detailed description of the scope of which is provided in Appendix No. 1 to the SWZ;
l) Rehabilitation/convalescence;
m) Stay for ICU,
2. additional packages (a detailed description of the packages can be found in Appendix 1 to the SWZ) available for employees (contribution financed by the employee) packages V1 to V3.
3. and additional packages (proposed by the Contractor as part of the offer evaluation criteria) for employees to choose from (contribution financed by the employee) packages V4 to V7.
