Description:
1. The subject of the order is the purchase of surgical robots - 2 pieces along with tools and accessories for performing procedures for Copernicus PL Sp. z o. o. The subject of the order consists of 1 part, which is divided into 3 tasks:
1) Task No. 1 Surgical robot I (1 pc),
2)Task no. 2 Surgical robot II (1 pc),
3) Task No. 3 Tools and accessories for procedures using the offered surgical robots (successful deliveries).
2. The detailed scope of the subject of the order, list of equipment, quantities and method of order execution are included in the annexes to the SWZ, namely:
1)App. No. 1A to SWZ Price form,
No. 1B to SWZ - Description of the Subject of the Order,
No. 3A to SWZ - Draft contract for a one-off delivery (concerning tasks 1 and 2),
No. 3B to SWZ - Draft contract for successive deliveries (concerning task 3).
The Contractor attaches Appendix No. 1A and 1B to the SWZ to the offer. The Contractor, by completing the indicated items in Annex 1A to the SWZ-Price Form, confirms meeting the requirements specified by the Ordering Party in task No. 3, and by completing the indicated items in Annex 1B to the SWZ-OPZ, confirms meeting the requirements specified by the Ordering Party in Task No. 1 and 2.
3. The offered items must be complete, of a high standard both in terms of quality and functionality, and free from material, construction and legal defects.
