The subject of the order is the purchase together with the delivery of medical equipment for item The order was divided into 6 parts (packages) according to the description contained in Annex 2 to SWZ - price form, Annex 2A - minimum technical parameters.
Actual documents together with the offer: 1. Offer form(Annex 1 to SWZ). 2. Price form (Annex 2 to SWZ). 3. Minimal technical parameters (Annex 2 A to SWZ) .4. Experience of the Contractor in the form of a uniform European order document (EAT) (Annex 3 to SWZ); 5. Document of the National Court Register or Cedig to verify the authorized personsto represent the contractor, thus making a declaration of will. In the case of an offer submitted jointly by several contractors, each contractor submits the above -mentioned document separately. 6. Power of attorney (if concerned) 6.1. When the authorization of the person submitting the offer does not result from the registration documents,The contractor, who submits the offer via a representative, should attach to the offer a power of attorney document signed with a qualified electronic signature. 6.2. In the case of contractors applying for the awarding of the contract, the Contractors are obliged to appoint a representative. Documentpower of attorney, from which the content will be resolved to represent these contractors in the procedure for awarding the contract, should be attached to the offer. 6.3.
