Description:
The subject of the order is the reconstruction of sanitary and electrical installations in the buildings of Mazowiecki Szpital Brodnowski Sp. z o. o. Attis branch at ul. Gorczewska 89 in Warsaw stage I documentation.
The subject of the order will be implemented taking into account the following Stages:
1) Stage I: Execution comprehensive inventory of existing buildings and design and cost estimate documentation of sanitary and electrical installations in the buildings of the Attis Center at ul. Gorczewska 89 in Warsaw, a branch of Mazowiecki Szpital Brodnowski Sp. z o. o.
2) Stage II constituting the Right of Option: Fulfillment author's supervision in the procedure(s) for the award of a public procurement contract for the performance of construction and assembly works (including, at the request of the Ordering Party - participation in the procedure(s) for the award of a public procurement contract as an expert);
3) Stage III constituting the right of option: Exercise author's supervision within the meaning of Art. 20 section 1 point 4 of the Construction Law (to the extent described in the draft contract, Annex No. 4 to SWZ), over the implementation of construction works covered by the completed Documentation.
Option right - the Ordering Party may exercise the option right regarding the implementation of works covered by STAGE II i III on the terms described in the draft contract (Appendix No. 4 to the SWZ) and the description of the subject of the order (Appendix No. 11 to the SWZ).
A detailed description of the subject of the order is attached as Annex No. 11 to the SWZ.
