Description:
1. The subject of the order is the performance of construction works regarding the renovation and reconstruction of 4 hospital facilities at the Independent Public Health Care Facility in Puławy in the design and build formula.
2. A detailed description of the subject of the order is specified in the Functional and Utility Program constituting Annex No. 2 to the Terms of Reference, hereinafter referred to as (SWZ).
3. The Ordering Party allows the submission of partial offers for the following parts of the order:
1) Part of the order No. 1 - Renovation/modernization of the General Surgery Department.
2) Part of the order No. 2 - Renovation/modernization of the Plant Endoscopy and adaptation of rooms.
3) Part of the order No. 3 - Renovation/modernization of the Oncology Clinic.
4) Part of the order No. 4 - Renovation/modernization of the driveway to Block D (ramp) along with the delivery and installation of an elevator.
4.Contractors may submit offers for any number of parts of the order.
5. In case unless the SWZ provides otherwise, all requirements apply equally to all parts of the order.
6. Requirements regarding the conditions for performing construction works.
When performing the subject of the order, the following Conditions must be observed in particular:
1) Works will be carried out inside and outside outside the active facility. The sanitary regime must be particularly observed. The performance of works must not disturb the functioning of the facility. The Contractor will adapt the most burdensome activities to the organization of work in the facility.
