- REQUEST FOR OFFER: Provision of medical services in the field of diagnostic tests for employees of RCGW S.A., in accordance with the provisions of the Description of the subject of the order (OPZ - Annex No. 1). The contractor is obliged to submit an offer form in accordance with the template of Annex No. 2 and the assortment and price form in accordance with the template of Annex No. 2A. The offer must be accompanied by, in accordance with point 11 of the OPZ: - entry in the register of entities performing medical activities kept by the territorially competent administrative authority, in accordance with Act of April 15, 2011 on medical activities (consolidated text: Journal of Laws of 2025, item 450, as amended). This procedure is conducted in accordance with the Regulations of RCGW S.A. in Tychy of February 6, 2024, regarding the award of public procurement contracts value below PLN 130,000.00 net. The contractor may submit only one offer in electronic form, with a valuation of the subject of the order described in the request for quotation within the set deadline. Failure to meet any of the above. conditions will result in the offer being rejected. The offer with the lowest price or presenting the best balance of price and other requirements/criteria described in the request for quotation is considered to be the most advantageous offer. Before selecting the offer, the Contractor allows the possibility of conducting price negotiations with the three Contractors who submitted the most advantageous offers that cannot be rejected.
