- REQUEST FOR QUOTE: Provision of medical services in the field of occupational medicine in accordance with the provisions of the Description of the subject of the order (OPZ - Annex No. 1). The contractor is obliged to submit an offer form in accordance with the template of Annex No. 2 and an assortment and price form in accordance with the template of Annex No. 2A. To offers must be attached, in accordance with point 7 of the OPZ: - the Contractor's declaration on providing the required parking spaces - at least 10 spaces; - the Contractor's declaration on the distance at which the services will be provided (i.e. all occupational medicine tests must be performed in the city of Tychy at a distance not exceeding 5 km from the headquarters of the Ordering Party, i.e. Al. MarszaΕka PiΕsudskiego 12 (calculated in a straight line according to https://www.google.pl/maps)); - entry in the register of entities performing medical activities kept by the territorially competent administrative authority, in accordance with the Act of of April 15, 2011 on medical activities (uniform text: Journal of Laws of 2025, item 450, as amended). These proceedings are conducted in accordance with the Regulations of RCGW S.A. in Tychy of February 6, 2024, regarding the award of public contracts worth less than PLN 130,000.00 net. Contractor may submit only one offer in electronic form, with a valuation of the subject of the order described in the request for quotation, within the specified deadline.
