1. The subject of the order is the transport of sanitary for the Provincial Specialist Hospital Of the Blessed Virgin Mary in Częstochowa. "
2. The order of the contract has been divided into 4 parts:
Part 1 -provision of patient sanitary transport services in a lying/sitting position.
Part 2 -provision of transport services for dialysis patients in a position from/to their place of residence from/to the Dialysis Center of the Provincial Specialist Hospital Blessed Virgin Mary in Częstochowa at ul. Bialska 104/118.
Part 3 -provision of sanitary transport services and blood -borne materials and blood samples of patients from/to the Regional Blood Donation and Blood Treatment Center in Katowice, ul. Raciborska 15 from/to the Provincial Specialist Hospital Blessed Virgin Mary in Częstochowa, ul. Bialska, ul. PCK 7.
Part 4 -provision of sanitary transport services for research materials and other materials Between the facilities of the Provincial Specialist Hospital Blessed Virgin Mary in Częstochowa.
3. The Employer allows for partial offers. The concept of partial offer is understood as individual parts of the order set in Annexes 2 to SWZ. The contractor may submit an offer for any part or on the entire subject of the order.
