1. The subject of the contract is the Sanitary Transport Service together with medical care for the Provincial Specialist Hospital Of the Blessed Virgin Mary in Częstochowa:
Part 1 - Transport by specialist team "S".
Part 2 - Transport by the basic team "P".
2. Detailed description of the subject Orders include:
1) Price form - Annex 2 to SWZ,
2) description of the subject of the contract - Annex 9 to SWZ,
3) planned provisions of the contract - Annex 10 to SWZ,
3. The applicant allows the submission of partial offers in accordance with the parts specified by the Ordering Party in Annex No. 2 Price form to SWZ. Each of the contractors can submit an offer for one part or more part of the order.
4. The applicant provides for selecting the best offer using the electronic auction.
