Description:
The subject of the order is the provision of services in the form of confirming death and its cause and issuing a death certificate, referred to in Art. 11 section 2 of the Act of January 31, 1959 on cemeteries and burial of the dead (consolidated text: Journal of Laws of 2025, item 1590), in accordance with with the provisions of the Regulation of the Minister of Health and Social Welfare of August 3, 1961 on the declaration of death and its cause (Journal of Laws of 1961, No. 39, item 202) and the Regulation of the Minister of Health of December 7, 2023 on the death certificate template (Journal of Laws Laws of 2023, item 2734), persons who died within the administrative boundaries of the city of Wabrzych in the event of the impossibility to determine the primary health care physician providing medical care for the deceased or the physician treating the patient with the last illness responsible for declaration of death. A detailed description of the subject of the order and the conditions of its implementation will be specified by the Ordering Party in the Draft Agreement constituting Annex No. 3 to the SWZ.
