- 1. The subject of the contract is: the service of group life insurance for employees and family members of employees of the Health Care Team "Poviat Hospital" in Sochaczew.
2. The contracting authority does not guarantee that all employed employees and family members of the Employer's employeeswill join group life insurance.
3. During the insurance period, there may be changes in the number of insured persons.
4. If the employee or member of the employee's family resigns from insurance, the contracting authority is not obliged to pay an insurance premium for them.
5.Detailed terms of the order can be found in Annex 1 to SWZ.
6. The professional structure of the Employer's employees is in Annex 2 to SWZ.
