Description:
1. The subject of the order is the servicing of medical equipment (part no. 1) and the lease of a washing station for flexible endoscopes along with the supply of chemicals and full service support (part no. 2). The subject of the order consists of 2 parts.
2. Detailed scope of the subject of the order, list devices, quantities and the method of order execution are included in the annexes to the SWZ, namely:
No. 1A Price form,
No. 1B Schedule for part No. 1,
No. 1C - Description of the Subject of the order for part No. 2
No. 3 Draft contracts.
3. Persons directed to perform the contract must: have qualifications specified by the manufacturer of medical devices.
4. The subject of the order must be consistent with the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws 2022.974 of 2022/05/09, as amended) - if applicable.
5. In all provisions of the SWZ and its annexes, in which: The ordering party refers to standards, approvals, technical specifications or reference systems or trademarks, parameters or sources of origin (names of manufacturers or devices) are indicated, in accordance with Art. 99 section 5 and art. 101 section 4 of the Public Procurement Law, the Ordering Party allows equivalent solutions.
6.W in the case of an assortment described by reference to standards, technical assessments, technical specifications and technical reference systems referred to in Article 101 section 1 point 2 and section 3 of the Public Procurement Law, the Ordering Party allows solutions equivalent to those described.
