Description:
1. The subject of the contract is service service of medical apparatus prod. Gambro at Copernicus PL Sp. z o.o. in Gdańsk. The subject of the contract consists of 1 part.
2. The concise scope of the subject of the contract, the list of devices, quantity and method of performance of the contract includes attachments to SWZ,Namely:
1) No. 1a - price form,
2) No. 1b - schedule,
3) No. 3 - draft contract.
3. The sets directed for the implementation of the contract must have qualifications specified by the manufacturer of medical apparatus.
4. Contractor in the provided partThe services are obliged to apply and comply with the legal provisions in force in the subject of the contract, have relevant rights and bears full responsibility in the event of their violation.
5. On the basis of art. 95 para. 1 of the Public Procurement Law, the contracting authority requires employment by the Contractor orSubcontractor based on an employment contract within the meaning of the Act of 26 June 1974 Labor Code Labor Code (Journal of Laws of 2019, items 1040, 1043 and 1495) continuously throughout the entire period of the contract of the contract of persons/persons performing service activities related to the implementation of the subject of the contract.
6.Before the conclusion of the contract, the Contractor shall transfer to the Employer a statement of compliance with the requirement referred to in para. 9, i.e. that a person/persons performing service activities related to the implementation of the subject of the contract will be employed under an employment contract within the meaning of the Act of 26 June 1974 CodePr...
