Description:
Part 1 Individual psychological consultations for residents of the Rawicz commune affected by or at risk of addiction problems, including behavioral ones1. Duration of service: on average 10 hours of consultation per month (minimum 1 consultation per week), greater than 120 hours during the entire implementation period.2. Conditions for participation in the procedure: In terms of technical or professional capacity (Article 116 of the Public Procurement Law): The contractor will demonstrate that he will assign a specialist to carry out the order who: 1) has higher education in psychology2) has at least 5 years of professional experience in conducting psychological consultations3) has a Psychotherapist Certificate of the Polish Psychological Association confirming the title of specialist in the field of psychotherapy of children and adolescents in the form of specified in the regulation of the Minister of Health of June 13, 2017. on specialization in fields applicable to health care (Journal of Laws of 2023, item 1771, as amended)3. Order completion date: from January 1, 2026 to December 31, 2026
