Description:
1. The subject of the order is the successive delivery of single-use endoscopic equipment for the needs of the Chorb Puc Odpowiedzi Specialist Hospital. Klara Jelska in Zakopane. Package No. 1 includes aspiration needles. The subject of the order is specified in detail in Appendix No. 1 (i.e. price calculation offers). 2. The subject of delivery is to meet the requirements specified in the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620), in particular it is to be marked with the CE mark, and if the conformity assessment was carried out under the supervision of a notified body, then next to the CE mark there is to be identification number of the relevant notified body affixed.3. The subject of the order is to be delivered in the manufacturer's packaging. 4. The outer and inner packaging must contain at least the name of the goods, batch number, production date and expiration date (if applicable).5. The contractor must guarantee accordingly long shelf life of the delivered products - min. 1 year from the date of delivery.
