Description:
Brief description of the subject of the order: The subject of the order is the successive delivery of medicines for the Provincial Rehabilitation Hospital. Dr. S. Jasiski in Zakopane. A detailed description and quantity of the subject of the order is set out in Annex No. 1 (i.e. price calculation of the offer) to this specification. 2) Ordering party inalso requires sending an electronic version of the invoice in EDI format (electronic data interchange). One template will be established at the beginning of cooperation and will not change until its end, which will allow for quick data import.3) The Ordering Party reserves that the Contractor's offer cannot include dietary supplementsy, in the case of using a dietary supplement, the Ordering Party will consider that the offer does not meet the conditions of the ToR and will be rejected.4) The expiry date (use by date) of the medicines supplied by the Contractor as part of the subject of the order must be at least 6 months from the date of delivery.5) Delivery goods are carried outor to the warehouse of the Ordering Party's Hospital Pharmacy. The goods will be collected by an authorized person. At the time of receipt of the goods, the employee will be obliged to examine whether the quantity and quality of the delivery is consistent with the attached documents and contract.
