Description:
1. The subject of the order is the successive deliveries of consumables and accessories intended for the Operating Room Clinics of the University Clinical Hospital. Fryderyk Chopin in Rzeszow.
2. A detailed description of the items of the order, estimated quantity and requirements are presented in the Forms prices and offers to Parts No. 1/3 (Appendices No. 1 to the SWZ), which are an integral part of the SWZ.
3. The scope of the order includes the delivery of the items of the order in the quantities specified in the Price and Offer Forms for Parts No. 1/3 (Appendices No. 1 to the SWZ) which are an integral part of the SWZ, by a period of 24 months from the date of conclusion of the contract.
