Description:
The subject of the order is the purchase and delivery of elements for osteosynthesis in the assortment and quantities specified in the Assortment and Price Forms constituting Appendix No. 1A to the Terms of Reference (SWZ).
The subject of the order has been divided into 9 parts - Packages.
Detailed description of the itemThe subject of the order is contained in Appendix No. 1 to the ToR.
The Employer allows the submission of partial bids for any selected packages. However, the Ordering Party reserves the right to submit offers by the Contractor for the entire assortment in a given package.
The subject of the order presented in the offer should have the documentsallowing its use in health care in the Republic of Poland in accordance with the requirements set out in the Act of 20 May 2010 on medical devices. The documents referred to above will be made available to the Ordering Party at its request.
Contractor in the scope of Package No. 1, 2, 3, 6, 8, 9 is obliged to create a bank of implants at the Ordering Party.
