Description:
1. The subject of the order is the purchase and delivery of implants and orthopedic devices in the assortment and quantities specified in the Assortment and Price Forms constituting Appendix No. 1A to these Terms of Purchase Specification (SWZ).
2. The subject of the order has been divided into 9 parts - Package3. A detailed description of the subject of the contract is contained in Appendix No. 1 to the ToR. 4. The Contracting Authority allows partial offers for any selected packages. However, the Ordering Party reserves the right to submit offers by the Contractor for the entire assortment in a given package.
5. The subject of the order presented in the offerrcie should have documents allowing its use in the health service in the Republic of Poland in accordance with the requirements set out in the Act of 20 May 2010 on medical devices. The documents referred to above will be made available to the Ordering Party upon its request.
6.WIn the scope of Package No. 1, 2, 4, 5, 6, 7, the contractor is obliged to create a bank at the Ordering Party
