Description:
1. The subject of this order is the successive deliveries of medical products to the National Institute of Oncology. Maria Skłodowska-Curie - National Research Institute, Branch in Gliwice.
2. Description of the subject of the order and terms of execution of the order are included in the annexes:
1)Specification assortment and prices Annex No. 2.1-2.27 to SWZ,
2) Designed provisions of the contract that will be introduced into the content of the contract - Annex No. 5 to the SWZ.
3. The Ordering Party allows the possibility of submitting equivalent offers - the description of equivalence is included in Chapter IV of the SWZ.
4. The ordering party does not demand from the Contractor to pay a deposit and does not require the Contractor to provide security for proper performance of the contract.
13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, the Ordering Party included in the SWZ.
7. Information on the joint application of Contractors for the award the orders were included in Chapter IX of the SWZ.
8. The Ordering Party provides for the possibility of changes to the provisions of the concluded contract (so-called contract changes based on Article 455(1)(1) of the Public Procurement Law) in relation to the content of the offer on the basis of which the Contractor was selected, in accordance with the conditions ...
