Description:
1. The subject of the contract is the successive delivery of disposable staplers, charges to stapler and sets of loads necessary to perform VATS procedures and open surgery procedures in the Specialist Hospital of Lung Diseases "Rebirth". The subject of the contract can be found in Annex 1 to SWZ (offer price calculation).
ensure (lend) for the duration of the contract:
tissues, referred to in items 9 and 10 of the table - price calculation to the offer;
11 Table - price calculation for the offer and tooling necessary to make 6 fugitives using disposable covers.
lent: instrumentation, electrosurgical platform - should take place within 7 days of placing the order. Earlier than after a month after the date of the last delivery of sets, covers purchased on the basis of the concluded contract.
to have entry or notification in the register of medical devices, issued for them a declaration of compliance and must be marked with a CE sign in accordance with the Act of 7 April 2022 on medical devices (i.e., Journal of Laws of 2024, item 1620).
