Short description of the subject of the order: 1. The subject of the order is the successive delivery of materials for completions and dental accessories; 2. The goods offered must: (a) meet the conditions for placing on the market and to use, provided for in the Act on Medical Articles of 20 May 2010 (Journal of Laws 2010 No. 107 item 679, as amended - hereinafter: the Act on medical devices) , confirmed by current documents (ie: complete applications or notifications to the President of the Office of Registration of medicinal products, medical devices and biocidal products, safety data sheets, CE certificate, declarations of conformity, certificates) - for those goods that require; b) have allowing documents them for trading and use in Poland - including CE certificates and documents confirming the fulfillment of these goods required by the law of standards - in the case of goods not subject to the provisions of the Act on medical devices; (c) for products qualified as cosmetics in accordance with the Regulation of the European Parliament and of the Council / EC / EC / No. 1223/2009 of 30 November 2009, have a notification in the portal of the Kosm ...
