Description:
1. The subject of the contract is the successive delivery of medical materials, lenses and medicines along with the lease of the apparatus for the ophthalmology department for the location in Wejherowo in 15 tasks.
2. Typical description of the subject of the contract, assortment and estimated quantities are given in Annex 2 and Annex No. 8 toSWZ.
Task 1 - Set for glaucoma operations
1. The subject of the order are the successive delivery of sets for glaucoma surgery for the needs of the ophthalmology department for the location in Wejherowo.
2. A detailed description of the subject of the contract, assortment and estimated quantities are given in Annex 2 to SWZ.
Task 2 -Materials and tools for vitrectomy along with the lease of the camera
1. The subject of the contract is the successive delivery of materials and tools for vitrectomy along with the lease of the apparatus for the needs of the ophthalmology department for location in Wejherowo.The quantity is given in Annex 2 and Annex 8 to SWZ.
