Description: 1. The subject of the order is the successive delivery of drugs to drug programs to the Municipal Medical Center. dr. Karol Jonscher in Łódź broken down into the following packages (with the possibility of folding partial offers in each package):
Package I - medicines for a drug program treatment of multiple sclerosis,
Package II - medicines for a drug program treatment for focal dummy and half-face face,
Package III - medicines for the drug program for the treatment of immunoglobulins in neurological diseases,
Package IV - drugs to the Leg Program of Neovascular (Exudow) Program. Figure of the degeneration of a spots associated with age (AMD). A detailed description of the subject of the order specifies Annex No. 7 to the SWZ.
2. Providing the subject of the order to the Pharmacy of the Municipal Medical Center dr. Karol Jonscher in Łódź (ul. Millionowa 14, 93-113 Łódź) will be implemented at the expense and risk of the contractor.
