Description:
The subject of the order is the successive supply of materials used to mechanical cerebral thrombertomy along with the creation of a deposit for the healthcare team in Bolesławiec. A detailed description of the subject of the contract includes a price form - Annex 2 to SWZ. Parameters described in the AnnexNo. 2 to SWZ as the required technical and utility parameters, they constitute a detailed description of the subject of the contract, the failure to reject the offer as incompatible with the terms of the order pursuant to art. 226 para. 1 point 5 of the Public Procurement Law. The contractor is obliged to create a deposit in placeindicated by the Employer (medical magazine), within 3 business days from the date of the contract. At the first delivery, the delivery of the deposit should be taken into account- in all sizes indicated by the Ordering Party. The deposit will be successively supplemented after each procedure with the set usedor its individual elements.
The contracting authority does not allow partial offers. Each contractor can submit only one offer for the entire order in writing, under pain of nullity. If the Contractor submits more than one offer, all offers of this contractorwill be rejected by the contracting authority. The contracting authority reserves that the offered subject of the contract must meet the requirements of the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974) and the Regulation of the European Parliament and of the EU Council 2017/745 of April 5, 2017....
