- The subject of the order is the successive supply of useful medical materials for the needs of the Ophthalmology Clinic of the Provincial Combined Hospital in Kielce. A detailed description of the subject of the contract and the amount of assortment ordered is specified in Annex 2 to SWZ - the assortment and price form.A list of the required technical parameters of the lending item for Package 1 is indicated in Annex 2A to SWZ. Information on the principles of delivery, warranty and remuneration are included in Annex 3 to SWZ - the proposed provisions of the Public Procurement Agreement andAnnex 3A to SWZ - template for a loan agreement for package 1.
Medical devices constituting the subject of the contract should be understood as medical devices within the meaning of the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620, as amended).
