Description:
The subject of the order is the supply of endoprostheses and other orthopedic implants.
A detailed description of the subject of the order is included in the form constituting Annex No. 1a to the SWZ.
If any trademark, patent or origin, source or specific process is indicated in the description of the subject matter of the contract, which characterize products or services provided by a specific contractor - it should be assumed that the indicated patents, trademarks, origin, processes determine the technical, operational and utility parameters, which means that the Ordering Party allows the submission of an offer in this part of the subject of the contract with equivalent technical, operational and utility parameters.
LIST OF SUBJECT MEASURES OF EVIDENCE
1. In order to confirm the compliance of the offered deliveries, the Ordering Party requests the submission of the following evidence together with the offer:
1) Declaration that the offered assortment has been admitted and placed on the market in accordance with the provisions of the Act of April 7, 2022 on medical devices and has current declarations of conformity and certificates of the notified body * content of the declaration included in the offer form
2) Manufacturer's and/or technique's data sheets operational and/or other manufacturer's materials confirming the parameters required in the description of the subject of the order, containing catalog numbers/groups of catalog numbers or other identifier of the offered assortment. *
