Description:
The subject of the order is the supply of endoprostheses, other orthopedic implants, bioactive synthetic materials for cat defects for a period of 24 months.
A detailed description of the subject of the order is included in the form constituting Annex No. 1a to the SWZ.
If indicated in the description of the subject of the order any trademark, patent or origin, source or specific process that characterizes products or services provided by a specific contractor - it should be assumed that the indicated patents, trademarks, origin, processes determine the technical, operational and utility parameters, which means that the Ordering Party allows the submission of an offer in this part of the subject of the order with equivalent technical, operational and utility parameters.
LIST OF SUBJECT MEASURES OF EVIDENCE
1. In order to confirm the compliance of the offered supplies, the Ordering Party requests that the above-mentioned supplies be submitted together with the offer below the relevant means of evidence:
1) Declaration that the offered product range has been approved and placed on the market in accordance with the provisions of the Act of April 7, 2022 on medical devices and has current declarations of conformity and certificates of a notified body * content of the declaration included in the offer form
2) Manufacturer's catalog cards and/or operational techniques and/or other manufacturer's materials confirming the parameters required in the description of the subject of the order, containing catalog numbers/groups of catalog numbers or other identifier of the offered product. assortment....
