Description:
The procedure is conducted as an open tender pursuant to Art. 132 of the Public Procurement Law. The subject of the order is the supply of medical devices for bariatric procedures according to the following parts: Task 1 - Task 7. Detailed description of the subject of the order above. parts have been demonstrated and describedny in the "Price Form" (Appendix No. 2) and in Template Agreement I (Appendix No. 3), Template Agreement II (Appendix No. 3a) and Template Agreement III (Appendix No. 3b) constituting an integral part of the SWZ. The contractual conditions for the implementation of the public procurement in question contain contract templates, which constitute an integral partpart of the SWZ. The estimated value of the subject of the order without option rights is PLN 2,723,921.00. In section II.1.5. the announcement provides an estimated total value taking into account the option law.
