Description:
The subject of the order is the successive deliveries of medicinal products used in drug and chemotherapy programs for SPZOZ WSS No. 3 in Rybnik. The delivery specification is included in Annex 1.1 to the Order Specification ("Price Form"). The remaining terms of the order are specified in the annex No. 2 to the Specification of the Order Terms, i.e. "Designed provisions of the contract", which will be introduced into the content of the contract/contract provisions. Agreements regarding the resolved packages will be concluded after the completion of this procedure with a validity period until December 31, 2026. Ordering Party allows the possibility of submitting partial offers, therefore the subject of the contract is divided into 5 parts (packages).
The Ordering Party allows the possibility of submitting offers for selected parts (packages), but does not allow the possibility of submitting offers for any selected items in a given part (in a given package), individual parts (packages) must include the full scope specified by the Ordering Party - if applicable.
139 of the Public Procurement Law, it will first examine and evaluate the offers, and then subjectively qualify the Contractor whose offer was rated highest in the given package, regarding the lack of grounds for exclusion and meeting the conditions for participation in the proceedings.
