- 1. The subject of the order is the supply of oncological drugs and various drugs specified in parts for the University Clinical Hospital. Fryderyk Chopin in Rzeszow.
2. A detailed description of the subject of the order and the Ordering Party's requirements are specified in the Price and Offer Forms - Attachmentconnectors No. 1 for individual parts that are an integral part of SWZ.
3. The scope of the order includes the delivery of the items of the order in the quantities specified
in the Price and Offer Forms for the period:
- Part from No. 1 to No. 31 - 12 months.
4. The Ordering Party does not allow the submission of offersequivalent.
5. ORDER COMPLETION DEADLINE:
Planned delivery of the items of the order:
- Parts from No. 1 to No. 31 - for 12 months from the date of conclusion of the contract.
In Part No. 1 to No. 31, the Ordering Party allows for the possibility of changing the validity period of the contract concluded with the Contractor in the event ofi.e. completion of the entire subject matter of the contract during its duration. In such a case, changing the contract requires signing an annex, provided that the Ordering Party unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the period for which it was concluded.
