Description:
1. The subject of the order is the successive delivery of medical and diagnostic equipment and medical products to the Hospital Pharmacy NZOZ of the District Hospital in Dzierżoniow Sp. z o. o. based in Dzierżoniow at ul. Cichej 1, 58-200 Dzierżoniow.
2. The subject of the order has been described in detail in Annex No. 1A to the SWZ - assortment and price forms and divided into parts presenting the forecast consumption for a period of 12 months.
3. Number of lots: 52 Lots with the possibility of submitting partial offers for each lot separately
