1. The subject of the order is: Supply of medicinal products and medical devices including materialsdressing, hemostatic, patient care products and specialized products used in ophthalmology for the needs of the WSK-2 PUM clinics. Description of the subject of the contract and estimated amounts of products were included in the unit price form (annex 2 to SWZ, hereinafter also asFj).
2. The removal requires offer
a). medical devices (in the scope of tasks 1-10)-within the meaning of the Act on medical devices (Journal of Laws 2024.1620, as amended), hereinafter: UOWM), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.
b). medical products in accordance withwith the Act of 6 September 2001 Pharmaceutical Law (regarding tasks 11-15)
c). cosmetics (applies to task 5 items 9 - 12)
3. The removal requires that the shelf life for all offered is a minimum of 12 months from the date of delivery to the ordering office.has the right to order a smaller amount of products, except that not less than 60% of the amount given in the unit price form.Fluorescein
