Description:
The subject of the order is the successive deliveries of medicinal products:
1) antiviral and thrombolytic
2) used as part of drug programs and
2) used as part of chemotherapy
described in detail in the assortment and price form, Annex 2A to the SWZ.
2. The Ordering Party allows possibility of submitting partial offers. total number of parts in this procedure: 22. all provisions contained in the SWZ regarding the offer also apply to the partial offer. if the SWZ does not contain a provision which part of a specific article, paragraph, section or condition applies, form, document, etc. means that it applies to all parts.
3. The Ordering Party specified the quality requirements relating to the subject of the order refers to at least the main elements constituting the subject of the contract - in connection with this fact, it uses the possibility indicated in Art. 246 paragraph 2 uPzp.
4. The conditions for the performance of the order are also specified in the draft contract in Chapter XX of the SWZ.
5. The Ordering Party requires that the offered products be:
a) medicinal products authorized for marketing in the territory of the Republic of Poland in accordance with the Act of September 6, 2001 Pharmaceutical Law,
b) listed in the current Notice of the Ministry of Health on the list of reimbursed medicines, foodstuffs intended for particular nutritional uses and medical devices in part C (drugs used as part of chemotherapy) - applies to Parts: 20, 21
