Description:
1. The subject of the order is the successive delivery of 6 laser protection lenses in accordance with the description and requirements contained in Annex No. 1 to these Specifications.
2. Offers that do not include the full scope of the subject of the order will be rejected.
3. Other terms of the orderspecifies the draft contract, constituting Annex No. 3 to the Specification.
Laser protection lenses 6 pcs.
1. Lenses compatible with the Lumenis Pulse 100H holmium laser, serial number: 68. Year of production: 2017.
2. A lens that protects against contamination of the internal laser optics, the so-called blast-shield, in case of fiber burning or other contamination 6 pcs
Additional requirements
3. CE Certificate or Declaration of Conformity - attach to the offer.
4. Contract duration: 24 months.
5. Successive deliveries during the contract period.
6. Delivery time up to 2 weeks from the date of placing the orderfrom an employee of the Supply Section.
7. Warranty min. 3 months (3 months 0 points, 4-5 months 5 points, 6 months and more 10 points).
8. Assessment criteria: 90% - lowest price, 10% - warranty duration.
