The subject of the order is the delivery and replacement of 12 pieces of IR-192 radioactive sources for the Microselectron HDR V3 sn 10852 device, disposal of used sources and servicing of the device used in the Clinical Department of Radiotherapy.2. I will order a detailed description of the itemis specified in Annex No. 1 to the SWZ Equipment configuration, list and scope of maintenance services, source delivery schedule.3. Place of order fulfillment: Clinical Department of Radiotherapy at the West Pomeranian Oncology Center in Szczecin at ul. Strzaowska 22.4. Code and name according to the Common Vocabulary of Orderse (CPV): 09344000-2 Radioactive isotopes.5. The subject of the order includes: 1) supply of 12 pieces of IR-192 radioactive sources for the Microselectron HDR V3 sn 10852 camera, 2) services related to the collection of used sources, their disposal, delivery of new ones along with replacement and service, in accordance with the manufacturer's procedure, 3) setst consumable parts for the Microselectron HDR V3 sn 10852 camera, for the duration of the contract, delivered as a set once a year.6.
