Description:
1. The subject of the order is a service consisting in the provision of transportation and pick-up services for disabled children and students to the Hope Social Rehabilitation and Education Center. The scope of the service includes in particular: a) transportation of disabled children (using wheelchairs) from their place of residence to the Nadzieja Special Rehabilitation and Education Center in Chem, together with the return journey after completing the classes and with care provided during transport and return from January 1, 2026 to December 31, 2026, b) the daily number of kilometers on the route, including the return journey, is approximately 59 km. 2. A detailed description of the subject of the order is included in: Appendix No. 1 to the SWZ. After concluding the contract (the contract template is attached as Appendix No. 2 to the SWZ), it will be attached to contracts.
