I. The subject of the order is: 1. The subject of the order is to transport participants from their place of residence to the Community Self-Help Home in Kkolewo. 2. The scope of the order includes: 1) transportation of disabled people by 9-seater bus from their place of residence a disabled person to the headquarters of the Community Self-Help Home in Kkolewo and back: the vehicle must be adapted to transport disabled people, the transport of people will cover a maximum of 15 people from the Leszno district and the city of Leszno - approximately 3,400 km monthly, i.e. approximately 40,800 km per year, the transport to the Community Self-Help Home in Kkolewo will take place from Monday to Friday from: 6:40 9:40, for departure between 1:00 p.m. and 4:00 p.m. (possibly postponed by approximately 30 minutes), the Ordering Party ensures during transport of the caregiver.2) driving a 9-seater bus belonging to the Community Self-Help Home in Kkolewo /Ford Transit Custom with registration number PL 9991H/ and transporting disabled people each time from the place of residence of the disabled person to the headquarters of the Community Self-Help House in Kkolewo and back: the transport will cover a maximum of 1...
