Description:
The subject of the order includes the transport and return of dependent and disabled people (including wheelchair users, a maximum of 2 people) residing in the City of Bdzin (maximum 40 people), as well as providing care for them during the journey, to two Day Care Homes, to which were referred (Day Social Welfare Home at ul. Rotmistrza Witolda Pilecki 7, hereinafter referred to as DDPS and Daily Welfare Home at the Senior Services Center at ul. 1 Maja 2, hereinafter referred to as DDP in CUS). The subject of the order includes systematic transport of dependent persons indicated by the Ordering Party disabled people on the home-home center route, in accordance with the daily organization at a given facility. The service will be provided on all business days in the period from the date of conclusion of the contract, but not earlier than from January 2, 2026 to December 31, 2026 (maximum 253 days depending on the date of conclusion of the contract), appropriately adapted by means of transport. A detailed description of the subject of the order is included in Appendix No. 4 to the SWZ.
