1. The subject of the order is the provision of services for the needs of the Provincial Specialist Hospital. Maria Skłodowska-Curie in Zgierz services:
Part I of transport of dialysis patients at the dialysis station of the Provincial Specialist Hospital. Maria Skłodowska-Curie in Zgierz in the period from on January 8, 2026 until January 7, 2028 24 months from the moment of signing the contract - task No. 1, in accordance with Annex No. 3a.
Part II transport of blood and blood products and biological material for research 24 hours a day, 7 days a week, 365 days a year by sanitary transport type K in the period from January 1, 2026 until December 31, 2027 -24 months from the moment of signing the contract, task No. 2, in accordance with Annex No. 3b.
1. Transport of dialysis patients at the WSS dialysis station in Zgierz
1. The subject of the order is sanitary transport services for patients dialysis, carried out 6 days a week from the place of residence to the Dialysis Station of the Provincial Specialist Hospital. Marii Skłodowska-Curie in Zgierz, ul. Parzęczewska 35, hereinafter referred to as the Ordering Party, and from the Dialysis Center to places of residence, in accordance with the schedule agreed with the Ordering Party:
a. Monday, Wednesday, Friday 7.00 a.m. - 11.00 p.m
b. Tuesday, Thursday, Saturday from 7.00 a.m. to 6.00 p.m
and also outside the schedule, if the Dialysis Center reports the patient's urgent transport (e.g. the patient has been canceled from the previously reported schedule and his health condition requires urgent dialysis...
