• Description:
• The subject of the contract is the delivery of a triple -related vaccine against measles - pigeon - rose - as described by the subject of the contract specified in the specification of the terms of the contract (SWZ) and on the conditions specified in the planned provisionsSWZ.
  Part 1: Triple combination vaccine against measles - pigs - rose.
  1. Triple combination vaccine against measles - mumps - a 140,000 dose rose.
  2. In question:
  2.1. based on art. 105 and 106 of the PZP Act in order to confirm the compliance of the offereddeliveries with the requirements, features or criteria specified in the description of the subject of the contract or the criteria for assessing bids or requirements related to the implementation of the order, the contracting authority requests the contractors to provide the following documents:
  2.1.1. Valid permission for admission permit as for the datefor the marketing of a medicinal product in the Republic of Poland issued by the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products
  or
  valid as at the day of submitting the offer permission to trade the medicinal product issued by the Council of the European Union or CommissionEuropean.
  2.2. Above The documents must have a validity period covering the entire period of order processing.
  2.3. The documents must be valid and valid as at the date of submission of offers.
  2.4. The composition of vaccines and marking of packaging must comply with the content of the documents referred to in point 2.1.
