Description:
The scope of the task is described in Appendix No. 6 to the SWZ. The data used to describe the subject of the contract are specified at the time of preparation of the procedure documents. As of October 27, 2025 There are 29 dogs and 4 cats in the shelter. The subject of the order should be performed by the Contractor in accordance with his knowledge medical, principles of professional ethics and applicable regulations regarding the provision of veterinary services, i.e.: - Act of March 11, 2004 on the protection of animal health and combating infectious diseases of animals (Journal of Laws of 2023, item 1075, as amended) - Act of December 21, 1990 about the profession of a veterinarian and veterinary medical chambers (Journal of Laws of 2023, item 154, as amended) - Act of December 18, 2003 on animal treatment facilities (Journal of Laws of 2019, item 24, as amended) - Regulation of the Minister of Agriculture and Rural Development of September 29, 2011 regarding the scope and method of keeping medical documentation - veterinary records of animal treatment and templates of this documentation and records (Journal of Laws of 2011, No. 224, item 1347) - Resolution of the City Council in Ary (adopted annually for each year of its validity) regarding the adoption of an animal care program be...
